to avail the treatment the concerned employees has to
bring an authorization / referral letter from the
empanelled organization. |

authorization / referral letter has to be submitted in
the corporate office on the 3rd floor of the OPD

In case of
emergency the patient has to bring his / her smart card
to the hospital |
