INSTRUCTIONS FOR SUBMITTING THE APPLICATION FORM
Please read the instructions carefully before filling the form.
 

Please read the prospectus and the instructions given below before filling the form.

N.B.: THE INFORMATION PROVIDED ON THE APPLICATION FORM WILL BE TAKEN AS FINAL.

1. Fill the “online application form” .You will need to upload a recent passport size photograph on the online application and click “submit”. If successful, a message will appear on the screen with the instructions.

2. Take a printout of the additional forms (form 99 and the application form)

3. Fill both the forms and paste a recent photograph (passport size) in the space provided (in the application form) and sign both the forms in the space provided.

4. Attach a Demand Draft for Rs. 3200/- drawn in the name of “Christian Medical College Ludhiana Society”

5. Send the following along with all required enclosures as per page 11 and 12 of the prospectus for MDS admissions 2012, to the office of the registrar mandatorily before 17^th February 2012:-

i.            Application Form

ii.            Form  99

iii.           Demand Draft
